If you have an Automatic Rollover IRA with Millennium Trust and would like to
receive a cash distribution, you will need to complete our
Automatic Rollover IRA Distribution Request Form and return it with a copy of your driver’s license.
You will need to get the form medallion signature guaranteed, a service your local bank should be able to provide,
if the distribution check is going to an address different than what we have on record for you. Also, if the address
on your driver’s license does not match, you will need to send in a copy of a current bill as well, showing
the address the distribution is going to.
If you would like to transfer your funds to another IRA account, you will
need to contact the receiving custodian and have them initiate their transfer paperwork. We require your
signature on this document to be medallion signature guaranteed. We will also need a copy of your driver’s license.
To roll your funds over to a qualified plan (401k, 403b, etc.) that you have established with your employer, you
will need to obtain a Rollover Form from your plan administrator or from your Human Resources department. This form
will outline for Millennium Trust, as custodian, what your plan requires in order to accept funds from an IRA. It will
include direction as to whom the check should be made payable, where the check should be sent and if they require a
copy of your current IRA statement, etc. You will also need to complete our
Automatic Rollover IRA Distribution Request Form, getting a medallion signature guarantee if the check is to go directly
to the plan, and return it with a copy of your driver’s license.
